Rehabilitation Services
Rehabilitation services are beneficial to our clients in assisting them to obtain their maximal goal of functioning. HHHS is happy to provide these services to our clients in the comfort and security of their own home.
HHHS physical therapists help patients suffering from injury or disease to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. In addition, they promote our client's overall fitness and health. HHHS physical therapists work closely with all members of our health-care team to ensure all aspects of our client's care are fully met.
Our physical therapists offer various solutions pertaining to our client's specific needs. These services include:

Meeting with physicians and all other home care professionals to understand overall goals for the patient with regard to physical mobility.

Consulting with patients and performing initial assessments as to the patient's current level of functioning and expectations for the future.

Developing plans for patients that will help increase mobility and ease of movement.

Assisting patients to learn to use various tools such as walkers, prosthetics, wheelchairs, or leg braces.

Using hot and cold temperature treatments, electric stimulation, massage therapy, and other treatments to help patients.

Completing all required documents and files regarding patient treatment and therapies.

Keeping all other home care professionals and other medical professionals informed of patient progress.

Implementing and performing various training programs/orientations and in-services to all home care staff.

Assisting with initial client assessment planning in conjunction with other health care professionals.
HHHS occupational therapists help patients who have limited abilities establish, develop, and/or maintain the ability to complete daily functions and tasks. Through regular therapy sessions, our occupational therapists work one-on-one with patients, focusing on their individual needs to help them achieve improved quality of life and a sense of independence. HHHS occupational therapy concentrates on ameliorating the physical, mental, social and emotional sources of our client's disabilities.

Evaluate patient's progress and prepare reports that detail progress.

Test and evaluate patient's physical and mental abilities and analyze medical data to determine realistic rehabilitation goals for patients.

Select activities that will assist individuals in learning life-management skills within limits of their mental and physical capabilities.

Recommend changes in patient's work or living environments, consistent with their needs and capabilities.

Consult with all other home care professionals to select activity programs and coordinate occupational therapy with other therapeutic activities.

Help clients improve decision making, abstract reasoning, memory, sequencing, coordination and perceptual skills, using computer programs.

Provide training and supervision in therapy techniques and objectives for home health aides, certified nursing assistants, and licensed and registered nurses.

Design and create, or requisition, special supplies and equipment, such as splints, braces, and computer-aided adaptive equipment.

Plan and implement programs and social activities to assist clients with maintaining daily activities.

Lay out materials such as puzzles, scissors, and eating utensils for use in therapy, and clean and repair these tools after therapy sessions.

Conduct research in occupational therapy.

Provide patients with assistance in locating and holding jobs.

Assist in implementing and performing training, orientations, and in-service sessions in conjunction with other health care staff.

Assist in implementing and assessing initial care plan assessments in conjunction with other medical professionals.
